This web-page is for you.  Once you schedule an appointment, you will be given a user ID and password to access your account on the patient portal.  At this secure site, you will be able to tell us about your pain condition on our patient questionnaire.  You can also fill out forms for demographics and insurance information.  As a returning patient, you will be able to check your current medications, request refills and fill out follow-up questionnaires.
